How does Dynaform24 work?

Forms for any purpose

Dynaform24 facilitates low-code building of various forms, for such purposes as:

  • Intakes and anamneses: make inquiries about the patient's condition.

  • Signaling plans: monitor a patient's condition to prevent relapses.

  • Medication monitor: anamneses and make inquiries about lifestyle.

  • Requesting copies of patient records.

An overall picture per patient

Submitted forms are linked to a patient record at all times. The healthcare data from the forms is stored in a standardised manner, based on openEHR information models (archetypes).

In this way, patient care information is accessible when necessary from a central, secure location: the EHR.

Dynaform24 is available as a module in conjunction with the mConsole EHR, or invoked through a single sign-on (SSO) integration.
